Main responsibilities, tasks, and abilities required:

• You will engage in a collaborative effort with our technical team to assess medication reconciliations, optimize patient medications, and validate discharge prescriptions using our state-of-the-art electronic patient record and prescription tool, PICs.
• As part of a larger clinical team, you will offer an expanded service to the Acute Medical Unit by aiding in the reconciliation of medications at the time of admission and ensuring that patients admitted to UHB receive exceptional care.
• In addition to aiding in the operation of our primary dispensaries as the Responsible Pharmacist, you will conduct expert screenings of clinic charts encompassing a broad spectrum of specialties.
