We espouse the home-first approach and the discharge-to-assessment methodology, firmly convinced that clients can preserve their autonomy and remain in their residences in the safest manner feasible with the provision of appropriate care. We foster a strong partnership with our inpatient rehabilitation providers and enablement teams to optimize client autonomy, promote self-assurance, and attain exceptional results. We collaborate with local Integrated Commissioning Boards (ICBs) as needed to ensure that the individuals we serve have access to funds for continuing care.

A comprehensive comprehension of the Mental Capacity Act (2005), Mental Health Act (1983), Equality legislation, and Deprivation of Liberty safeguards is essential for this role. This legislation must apply to your daily practice. Additionally, you will be obligated to fulfill the safeguarding responsibilities outlined in section 42) of the County’s regulations, which aim to prevent or reduce potential harm to the clients we cater to. It is expected that you conduct client assessments in adherence to our additional legislative obligations, including section (9) assessment under the Care Act (2014). This entails utilizing a strength-based, outcome-oriented approach. While prior experience in the court of protection work is preferred, successful candidates will receive training for this position.
